OCJAAF Volunteer Background Check

Team Managers and Head Coaches: Please have your Game Day Compliance Agents, Roster Staff, and any other volunteer that carries a badge complete the background check. Please tell them that they only need to do this ONCE; the background check will go towards any badge or title that they hold. All background checks must be done prior to issuance of a picture badge for any volunteer of MVYF and OCJAAF.

2011 OCJAAF Incident Reports

We will provide a link to the 2011 form as soom as it is available. These OCJAAF incident reports are to be completed for every injury that occur on the fields. Please give completed forms to the Chapter President within 72 hours of the injury so that proper insurance paperwork can begin.

Check Reimbursement Request

Please make sure you confirm with your Coach before spending any money out of your team funds account. Only money spent on expendable items specifically for the children on that team will be reimbursed. Please ask first If you have any questions about an item(s) that you will request reimbursement for. Check request forms will only be accepted by the Team Mom/Dad or Head Coach.
